Health insurance for administrative staff:
- Submit an application to subscribe with the health insurance for employees at the Hashemite University within (60) days from the date of starting work and getting the employee job number.
- If the specified period 60 day has finished, the employee has not the right to subscribe with the health insurance , and he/she can submit another application in the next year.
- One JD will be deducted from the employee’s salary, for each issued health insurance card.
- The application must be attached with the following documents:
1. A copy of valid family book, or a photo for the civil status identity for single person.
2. A personal photo for the employee and the individuals will be subscribed with the health insurance.
3. A document of student confirmation for the students who are above 18 years old.
Adding beneficiaries:
• Adding the wife:
- An application to add the wife is submitted within 60 days of marriage date
- A copy of the family book, or the marriage paper is attached
- One personal photo
• Adding newborns:
- An application to add the newborn is submitted within 60 days of birth
- A copy of family book, or the birth certificate must be attached to the application
- In the case of being late of submission, the employee can apply in January of the next year
• Adding parents
- The applications of adding parents is submitted in the beginning of every year, the applications is shown to a committee for studying it, providing that:
- The parents should not benefit from other health insurance
- The employee must be the only one who handle the expenses of the his/her parents
- Signing the commitment related to this purpose
- Filling out the form of adding parents
- The application must be attached with the following:
. a copy of valid family book
. personal photo
. a document from the institution of social security proofs that the parents do not receive a retirement salary
A document from the Ministry of Finance proofs that the parents do not receive ant civil or military salary
The percentage which the employee pays of the treatment expenses:
- (00%) of the treatment expenses in the hospitals and centers of the Ministry of Health
- (10%) of the treatment expenses in the hospitals of the Royal Medical Services
- ( 10%) of the treatment expense in the hospitals of Jordanian Universities
- (20%) of the treatment expenses in the private hospitals
- ( 25%) of the treatment expenses in the areas where there are no accredited private hospitals
- (75%) of the treatment expenses in the non-accredited private sector.
The percentages which the parents and the spouse of the participant hold from the treatment expenses:
- (20%) of the first one thousand JD of the annual treatment expenses
- ( 30%) of the second one thousand JD of the annual treatment expenses.
- ( 40%) of the total amount of money which exceeds two thousand JD of the treatment expenses.
The list of the accredited hospitals which provide treatment services to the employees of the Hashemite University:
- All of the hospitals and centers which related to the Ministry of Health
- All of the hospitals and centers related to the Royal Medical services.
- The health center inside the University’s campus
The following treatments are excluded from the health insurance:
- Cosmetic surgeries , cosmetic materials and its related preparations
- Treatments of infertility and contraception
- Treatments of artificial limbs unless it was caused by an accident
- Hearing tools and all of its types
- Vaccines and all of its types
- Vision correction through all of its means
- Dental treatment, except extraction, silver fillings, treatment of dental canals and surgical operations including its types, treatment of gum illnesses and tissue around the teeth
- Judicial accidents
- Physiotherapy outside the accredited hospitals
- Organs transplantation outside the kingdom of Jordan
- Tilt table examination
- Therapeutic shampoos
- Eye treatment with laser and Lasik and treatment of keratoconus
- Liquid nitrogen treatment
